  • The following is a list of the most important criterion used by CAP Pharmacy, LLC (CAP) to measure performance and improve productivity and overall Quality Improvement (QI).

    Inventory
    • CAP maintains a comprehensive inventory of products commonly used in the LTC setting. This includes oral, injectable and IV meds.

    • CAP has a list of preferred drugs for use for managed care plans.

    • CAP has a secured area for physical storage of drugs, with necessary added security as required by Federal and state law for controlled substances.
    Pharmacy Operations and Prescription Orders
    • CAP pharmacists are trained geriatric clinicians to meet the requirements for dispensing prescription drugs to LTC residents.

    • CAP conducts routine drug utilization review (DUR) for all orders to screen for allergies and drug interactions.

    • The CAP pharmacist will identify potentially adverse drug reactions, to identify inappropriate drug population and to promote cost effective therapy.

    • CAP is equipped with pharmacy software sufficient to meet the needs of prescription drug ordering and distribution to an LTC facility.

    • CAP will provide written copies of the policy and procedures manual at each nursing station.

    • CAP will provide ongoing in-services and training to make the nursing staff proficient in the process for ordering and receiving medications.

    • CAP will be responsible for return and/or disposal of unused medications following discontinuance, transfer, discharge, or death of a resident as permitted by State and Federal Law.

    • CAP has the capacity to provide specialized drug delivery formulations to facilitate effective drug delivery (i.e., via suspensions, gels, split pills or crushed pills) to LTC residents who may be unable to swallow or ingest medications through normal means.

    • CAP will schedule routine delivery of medications to the LTC facility as required by the facility to meet their needs. Emergency delivery service will be made as necessary 24 hours a day, seven days a week.
    Special Packaging
    • CAP has the capacity to provide special packaging commonly required by LTC facilities.

    • CAP can furnish other medical supplies and/or equipment as required by residents, upon request.
    • CAP has the ability to provide sterile IV medications, supplies and infusion pumps.

    • CAP has IV trained pharmacist’s and technicians as required to compound and dispense sterile products.
    Pharmacist’s On-Call
    • CAP has a pharmacist on-call 24 hours a day, seven days a week to handle calls after hours and to provide medication dispensing during an emergency, holidays, and after hours of normal operations.
    Emergency Kits
    • CAP will provide an “emergency” supply of oral, injectable, intravenous and refrigerated medications. This is supplied in the form of a computerized, electronic, dispensing device if the facility desires.

    • CAP will provide a system for logging and charging medication used from the emergency kit. CAP maintains a comprehensive record of a resident's medication order and drug administration profile.
    Miscellaneous Reports, Forms and Prescription Ordering Supplies
    CAP will provide reports, forms, and prescription ordering supplies necessary for the delivery of quality pharmaceutical care. Such reports, forms, and prescription ordering supplies will include provider order forms, monthly management reports to assist the LTC facility in managing orders, medication administration records, treatment administration records, interim order forms for new prescription orders, and folders for order storage and reconciliation in the facility.
